When admission to the hospital is necessary, your primary care physician may refer your care to a hospitalist. Once admitted to St. Mary’s Sacred Heart Hospital, the hospitalist will be in charge of your care and see you daily. Since hospitalists do not maintain an outpatient practice, they are more readily available to respond to hospital patients’ lab reports, test results and emergency situations. By also maintaining close relationships with referring physicians and other medical professionals, hospitalists provide an added level of continuous care with enhanced availability to you, family members and your primary care doctor.
What happens when a patient is admitted?
The hospital team of physicians will work with your primary care physician, specialists, nurses and other clinical staff to manage and coordinate your inpatient experience. If you do not have a primary care physician, hospital case management will assist you and your family to coordinate follow-up care after discharge.
